    1. If the application meets all requirements, a license shall be issued within 14 days of the application date.
    2. Use the address from your driver's license.
    3. Have you lived at this address for longer than a year?*
    4. Please list all places of residency over the past year and include city, state, and zip code.
    5. List the name of the company or association you are soliciting for?
    6. Have you worked with your employer or been affiliated with the association listed for longer than a year?*
    7. Please list all places of employment during the preceding year.
    8. If you are an applicant from out of town please list your local field office (aka Hotel) you are working from.
    9. Please list the nature and character of the goods to be sold or services to be rendered by the Applicant. Include the brand, logo, etc., or the purpose for which funds are being peddled or solicited.
    10. Have you recently conducted peddling or solicitation activities in other municipalities?*
    11. The following MUST accompany this application:
      The applicant shall furnish a photographic identification card and photograph taken within one (1) year of the date of application. Please fax photo IDs to (513) 539-6209. The applicant shall also provide verification that he/she or his/her employer has complied with the requirements of Ohio R.C. Chapter 1716 pertaining to charitable soliciations, if applicable; and all persons acting on behalf of any recognized not-for-profit charitable organization shall provide a statement attesting to the organization's exempt status under Section 501(c)(3) of the Federal Internal Revenue Code.

    14. To pay by check, money order, or cash
      Please mail or deliver your payment to the City of Monroe at 233 South Main St., Monroe, OH 45050. Your payment must be received in order for your application to be processed.
    15. By signing this document I acknowledge that my application will not be processed until payment has been received and the permit shall be issued withing 14 business days of the completed application.
