Complete the information below if you are the new renter at a residential or commercial property, and you want to establish an account for utility services.
All renters are required to submit a copy of their lease agreement and pay a rental deposit of $200.00. The lease can be uploaded at the end of this application process. The deposit can be paid by phone. Please note that your application will not be processed until the lease and deposit are received.
Please remember to include the unit number, if there is one.
I agree to be bound by the provisions adopted by the City of Monroe, Ohio, as they exist currently or as they are amended. Upon vacating the premises, I agree to provide proper notice (at least one business day) to the Utility Department. I understand that the total balance displayed on the final bill must be paid by the due date on the final bill.I authorize access to the water meter for all purposes deemed by the City of Monroe.
As the owner of the service address, I understand and agree that I am liable for all outstanding utility charges billed to the property. I also understand that delinquent balances may be certified to the property taxes if they remain unpaid.
NOTIFICATION OF TAX FILING REQUIREMENTAll residents living in Monroe (including part-year residents) are required by the Ohio Revised Code Section 718 and Monroe Ordinance No. 2015-40 to file a local tax return. A return is required to be filed even if tax is fully withheld. Retired residents are only required to file if they make more than the standard deduction for each specific year.Please complete the resident account registration form and submit it with your annual return. This form may be found on the City website, or you may call the Income Tax Department to obtain the form.TAX DECLARATIONUnder penalties of perjury, I declare that I have examined the information on this form, and closest to my best knowledge and belief, the information provided herewith is true, correct and complete.
